Effective immediately: the Certificate of Service attached to the Subpoena form (WCC Form H-08) will contain the following text: "Note: Certificate of Service only required for Medical Records Pursuant to Annotated Code of MD, Health General Section 4-306(b). The Commission will continue to use the remainder of the current forms with the stamped  notation until the form is reordered.

Thereafter, we will use the new  Subpoena form which is now posted on the Web (The Certificate of Service is available as a separate form only from the web page). The public may use either version of the Subpoena at this time, either the paper form supplied by the Commission or the form downloaded from our Website.
