indicated on the form. The new form is revised and available for
electronic filing through Online Services subscribed Claimant Attorney/Proxy. The
fillable PDF form on our
Forms and Instructions page is also updated. The Commission does
not accept form filing via email/attachment or fax.

opioid crisis being faced by the workers’ compensation
community. Panelists include medical professionals from
the pain management and rehabilitation community,
Commissioners, and attorneys who will speak on the issue
and lead us in discussions with an eye toward a
