Important CMS-1500 form
version 02/2012 Notice - April 1, 2014: Payers
will begin to receive and process paper claims submitted
ONLY on the revised CMS-1500 form (version 02/2012).

Effective October 1, 2013, an
employer that fails to secure compensation (workers'
compensation insurance) for all covered employees may be
subject to a penalty not to exceed $10,000. See Labor &
Employment 9-407
(Labor and Employment 9-407 Failure to Insure).
