30 The WCC fillable PDF form Request to Implead a Party - (WCC H33R Rev 10/2008) is revised to reflect the new address for the Maryland Subsequent Injury Fund (SIF).  Please discard any saved versions of the fillable PDF form and use the new form posted to our Forms & Instructions page.

Effective 5:00 p.m. August 22, 2012 the claimant's Social Security number (SSN) is a REQUIRED field to View Claim Documents and use the First Report of Injury search (available to some subscribers).

The SSN entered MUST BE VALID, as an example, entering 000-00-0000 or 999-99-9999 will not grant access to view documents in the WFMS system, no wildcard option can be used in the SSN field.

This affects only document viewing services; Claim Inquiry, File Forms, Hearing Issues, Hearing Results, Award Inquiry, etc. are unchanged.  The Public Claim Data Inquiry function on our home page is not impacted. Vocational Rehabilitation form services are not affected.

It is suggested that subscribers use the basic Claim Inquiry search to help to identify any prior claims by name and date of birth when the Social Security number is unknown or unavailable.

This additional security is required to secure non-public information in the View Claim Documents service and resultant recent abuses of our web site intended to access WCC information for unauthorized purposes.

Research on claims not accessible due to these changes may be facilitated by visiting Public Service at our main office in Baltimore City during normal business hours. Questions about onsite viewing can be directed to Public Service via email: [email protected]


 

Video equipment in our hearing rooms supports only DVD or VHS tape. Other media or input devices, e.g. CD, USB/storage devices, or video formats e.g. MP4, WMV cannot be used by our hearing room video equipment,  


20 ATTENTION PRACTITIONERS:
The Commission's Request for Document Correction form (C90R) may only be used for undisputed factual errors that exist in a document that has been filed in a specific workers' compensation claim. If a factual dispute exists, the dispute should be resolved at a hearing.  Do Not Use a Request for Document Correction to resolve a disputed matter.

Also note that Settlement Agreements submitted to the Commission for approval must include all parties in the settlement heading, regardless if all of the parties are settling. If a claim is being settled with parties that are not listed in the case, you must resolve this issue before submitting the settlement.
